Conditions

Tissue biology first, device second

Each condition raises a different biological question. The curriculum connects mitochondrial stress, inflammation, vascular change and gland structure to objective clinical findings.

Intermediate disease

Dry Age-Related Macular Degeneration

Until recently the honest answer to the dry AMD question was supplements and observation. Recently published trials changed that: 51% of treated patients gained at least one line over two years, with measurable drusen reduction - and, for the first time, no geographic atrophy forming beneath resolving drusen.

What helps measure it

  • Baseline OCT with drusen assessment
  • Best corrected acuity documented against the trial range (20/32 to 20/70)
  • Age and candidacy review against the published trial criteria (50 or older, no central geographic atrophy)

Clinical relevance: Photobiomodulation is considered in relation to mitochondrial support, oxidative stress and inflammatory signaling, with OCT, acuity and drusen change used to evaluate response over time.

Managed conservatively

Geographic Atrophy

Lesions can appear to grow early in treatment, but the expansion rate slows markedly against untreated progression. The target is the inflammation at the border between atrophied and healthy tissue, where complement cascade activity drives lesion growth.

What helps measure it

  • OCT with geographic atrophy lesion mapping
  • Energy-exposure planning per eye, including patching schedules when one eye carries more risk
  • Dermatology and photosensitivity review

Clinical relevance: The clinical focus is the metabolically active border between atrophied and healthy tissue, with serial imaging used to distinguish tissue response from untreated progression.

Restaged at every entry point

Evaporative Dry Eye

Evaporative, aqueous-deficient and mixed presentations do not respond to the same sequence. Restaging first is what makes device outcomes reproducible - and photobiomodulation now covers the presentations IPL physically cannot reach.

What helps measure it

  • Symptom questionnaire and tear film assessment
  • Meibography, or meibomian gland imaging, with dropout and expressibility grading
  • Lid margin and Demodex evaluation

Clinical relevance: Therapy is selected by the vascular, inflammatory and glandular findings. Photobiomodulation can reach upper-lid and lash-line tissue that a limited IPL treatment field cannot.

Obstructive and inflammatory subtypes

Meibomian Gland Dysfunction

Gland structure, not symptom score, determines how much recovery is realistic. Set that expectation before the first treatment.

What helps measure it

  • Meibography, or meibomian gland imaging, with dropout grading
  • Expression quality by gland region
  • Blink completeness assessment

Clinical relevance: Gland structure, inflammatory load and meibum quality guide how IPL, RF and home therapy are considered together.

Chronic, relapsing

Ocular Rosacea and Lid Margin Disease

Lid margin vasculature is the target. Patients need a maintenance plan, not a one-time course.

What helps measure it

  • Lid margin telangiectasia grading
  • Skin type and photosensitivity screening
  • Dermatology co-management review

Clinical relevance: IPL with defined maintenance intervals and documented consent.

Beyond the traditional critical period

Amblyopia

A 2012 German study reported improvement in 90% of treated amblyopic eyes, with half gaining three or more lines - well past the classic critical period. Dr. Brothers has replicated the pattern, including a 17-year-old moving from 20/100 to 20/40 and then 20/25 across treatment cycles.

What helps measure it

  • Best corrected acuity in each eye, tracked visit to visit
  • Refraction and anisometropia review - anisometropia has resolved in several treated hyperopes
  • Multifocal ERG where objective confirmation of retinal function change is wanted

Clinical relevance: Acuity and, where appropriate, multifocal ERG help assess whether light exposure is associated with functional retinal change beyond subjective perception.

Progressive myopia

Myopia Control

Repeated red light therapy outperformed diluted atropine and orthokeratology on axial-length change in comparative trials - making it one of the more compelling options in the myopia control conversation.

What helps measure it

  • Axial length and cycloplegic refraction baseline
  • Prescription stability review at six weeks of treatment
  • Home device safety briefing with timer verification

Clinical relevance: Axial length and refraction are followed to evaluate the response to red light therapy, with device safety and prescription stability kept central to clinical decisions.

Cases in context

What the condition reveals about tissue response

A longitudinal dry AMD case

A patient followed from December 12, 2024 through eighteen months of acuity and imaging change. Both eyes reached 20/20 by six months, one drusen cluster had resolved, and later imaging showed continued central and paramacular clearing.

Full-face treatment planning

Why Dr. Brothers extends IPL beyond a limited under-eye field when rosacea-associated vessels continue across the cheeks and jawline, considering the dry-eye and aesthetic effects together.

Sleep, environment and lifestyle

How sleep quality, fans, CPAP, screen use before bed, sleep masks, eye drops and diet can influence dry-eye symptoms or how well treatment gains are sustained.
